New patients, please call the clinic to schedule an appointment. We cannot utilize this form until your demographic information is entered into the electronic health record.

• Established patients may use the appointment request below. 

• Scroll ahead to a future week by clicking on the small arrow next to the dates at the top of the schedule. Dates with no open times will appear blank. 

• Once you choose a date and time from the calendar, you will move to step 2. Once the prompted information is entered, click the arrow on the far right. This will bring you to step 3 where you will enter your name and contact info.

• Click "Finish" on the right hand side of the window. This will send your appointment request to the Mankato Integrative Medicine's schedule for approval.

• Once your appointment request is approved, you will receive an email confirmation. If we cannot accommodate your requested time, the office will call or email you to find a workable time.
